Dear Friends in Christ:

It’s that time of year again when we reinforce our understanding that we are all a part of a connectional church, The Presbyterian Church (U.S.A.), or, in other words, we are asking you to help us paint a picture of our greater church by completing the On-Line Annual Statistical Report for the Year 2011 and providing the other information noted below.

Presbyterian Church (U.S.A) is no longer mailing hard copies of the Instruction Workbook for completing the On-Line Annual Statistical Report for the Year 2011 or the Clerk’s Annual Questionnaire for Year Ending December 31, 2011 to the presbyteries. You may download the following items from the Mission Presbytery website www.mission-presbytery.org under the Resources tab, then click on Forms.

1. The Instruction Workbook (English, Spanish or Korean versions available) for completing the 2011 On-Line Annual Statistical Report

Item 1. 2011 On-line Annual Statistical Report. The deadline for submission of the on-line report is Wednesday, February 22, 2012. We strongly recommend you complete your input a week or two earlier to avoid the frustration of encountering a very busy, sometimes overloaded system. If you or your church does not have on-line access, please contact the Presbytery Office for assistance. The on-line system will be available beginning December 7, 2011. If you experience difficulty accessing or using the system, remember there is a hotline you can call at 1-888-728-7228 x 8144.

Item 2. Clerk’s Annual Questionnaire for Year Ending December 31, 2011 may also be completed on-line and should be completed by Wednesday, February 22, 2012. If you do not have on-line access, please contact the Presbytery Office.

Item 3. Clerk of Session Information Form, should contain the latest contact information for your clerk of session. This is the main point of contact between the Presbytery office and each church and current and complete information is a great help. Even if there is no change from last year, we ask that you complete this form and return it to the Presbytery office by January 31, 2012.

Item 4. Necrology Report, is used to help us celebrate and remember the lives of those elders and deacons in the church who died during calendar year 2011. This celebration will take place at our March 2-3, 2012 meeting of Mission Presbytery at Northminster, San Antonio, so please return it to the Presbytery office by January 31, 2012.

Item 5. Minimum Terms of Call. Effective January 1, 2012 as reported at October 2011 Presbytery meeting.

Item 6. Report of 2012 Changes in Installed Teaching Elder Terms of Call to the Committee on Ministry, reflects your congregation’s vote approving your Installed Pastor’s terms of call for 2012. The Book of Order requires that any changes to terms of call must first be approved by the congregation and then be approved by the Presbytery. This will be accomplished at the June 8-9, 2012 meeting of Mission Presbytery at First Presbyterian Church, Corpus Christi; therefore we need your completed terms of call document by March 31, 2012.

Note: Terms of Call Reports are needed for installed teaching elders (pastors, co-pastors, associate pastors, and designated pastors) only.

Temporary pastoral relationships such as stated supply, organizing pastor, interim pastor, interim co-pastor, interim associate pastor, temporary supply and Commissioned Ruling Elder do not require congregational action or submission of a “Terms of Call” form, but an annual contract/covenant between the session and individual is required. The contract/covenant must be submitted to, and approved by, the COM annually.
